PATH TEAM

Overview

The PATH program is a unique collaboration between the Tri-County service providers, led by Housing Services Mid Michigan, to find and assist the area's homeless population in securing shelter options which will lead to permanent housing possibilities. This consortium of service providers is focused on building trust, combining their services using a strengths-based short-term care model and working together in the best interest of mutual clients.

Program Aspects

  • Face to face interactions with people living on the streets, vehicles, camps, or in other non-traditional settings unfit for human habitation.
  • Engage clients and connect them to services
  • Coordinate other MDHHS resources that support housing stability including benefits such as Medicaid, food assistance, children's services, behavioral health, disability services, and employment and training programs.
  • Connect homeless individuals to the Housing Assistance Resource Agencies (HARA) using coordinated entry process.
