Homeless/Eviction Appointments

Items to Bring to Appointment:

  • Proof of income for the last 30 days (paycheck stubs, award letter, or bank statement showing direct deposits)
  • Proof of emergency (court judgment, homeless shelter verification, letter from people you are staying with)
  • Photo ID for everyone over 18 in your household
  • Social Security cards for everyone in your household
  • SER Decision Notice
  • Information about monthly expenses (food, utilities, gas, etc.)

Please Note: An appointment with us does not guarantee financial assistance with your housing emergency. If HSMM can assist financially, it will only be with a portion of the total amount. Here are a few other agencies that may be able to assist:

Public Notice for data collection

HARA Intake App Form: If you have already scheduled an appointment with a case manager, please complete this application and bring it to your appointment.

Landlord Form: This form needs to be completed by your landlord and returned to your case manager.

Income Verification Form: If you recently started a new job and are unable to provide proof of income for the last thirty days please have your employer complete this form and return it to your case manager.

 

Foreclosure Appointments

Items to Bring to Appointment:

  • Proof of income for last 30 days (pay stubs, award letter, etc.)
  • Bank statements for last 30 days
  • Most recent mortgage statement
  • Most recent property tax statement (if applicable)
  • Any communication from mortgage company or attorneys for the mortgage company
  • Budget documents (utility bills, other expenses)

Foreclosure Intake Application: If you have already scheduled an appointment with the foreclosure specialist, please complete this application and bring it to your appointment.
